Ongoing Health Needs

We evaluate and help manage common chronic concerns such as high blood pressure and diabetes, with monitoring, lab work, medication management, lifestyle counseling, and referral when appropriate.

Your First Visit+

Bring a photo ID, insurance card, medication list, allergy list, and relevant medical records. If transferring care, ask your previous practice how to send records securely. Confirm appointment availability by phone or through the scheduler.

Care Beyond Our Clinic+

Some symptoms, diagnoses, tests, or treatments require a specialist, imaging center, hospital, or emergency department. We will explain when a higher level of care or referral is appropriate.
